THE QCHR HEALTH HUB
The QCHR Health Hub is a welcoming space where participants can access essential supplies and supportive services. Here, individuals find harm reduction materials, food, clothing, hygiene items, and guidance with social service navigation such as IDs, insurance, and benefit applications. More than a supply site, the QCHR Resource Center offers dignity, connection, and practical support to help people overcome barriers and move toward stability.

Through QCHR, Center for Prevention Services connects individuals seeking substance use treatment or medication for opioid use disorder (MOUD) with qualified, trusted providers and walking them through the first steps of care.

Center for Prevention Services links participants to social service agencies and helps them access government aid; obtain IDs and Social Security cards; apply for disability benefits; and secure insurance coverage.

REFERRALS
The QCHR team is available to connect individuals in need with trusted healthcare providers, while also scheduling appointments and attending initial medical visits to ensure they feel supported and understood. ​

Through individual advocacy, Center for Prevention Services is turning empathy into action and dignity into hope. We strive to ensure that all participants have access to essentials like food, clothing, shelter, and kind, compassionate care.
HOUSING FIRST
INITIATIVE
This program provides safe, stable housing for qualified participants experiencing chronic homelessness to support recovery and long‑term stability. Due to high demand for these services, the program currently has a significant waiting list.
